Moral of the story – protect yourself and rule out the worst disease possible (i.e. the one that will get you in trouble if you miss it). The patient in this case saw the physician six times before the cancer was found. After the third or fourth visit of persistent symptoms, an upper endoscopy would not have been unreasonable to ensure there wasn’t anything more serious going on.
