I see this a lot.&lt;br/&gt;&lt;br/&gt;What is also a problem that I see a lot more now are physicians who swap the patient off from one to the other with no one taking responsibility for making sure that what needs to get done gets done.  The idea always has been, and still is in my book, that the attending physician bears total responsibility for  making sure what needs to get done gets done--&quot;it&#039;s not my job&quot; is never an excuse.  Everyone at the hospital from the social worker to the ward clerk are there to help you do  your job--but it is still  your responsibility.
