The error was with mixing in the pharmacy. It appears the pharmacy staff involved have taken &quot;voluntary&quot; leave. I really hope this is voluntary. Errors can occur but someone should not get fired unless they are grossly negligent or fail to learn for previous educational measures. The system itself seems flawed as why would pharmacy be making batches of heparin flush...this is a big no no. Unless, they were forced to do it because of a heparin shortage.
