Would computerized order entry have helped the Texas case? Probably not.

Neither physician order entry or beside barcode technology could have prevented this error. The error was with mixing in the pharmacy. It appears the pharmacy staff involved have taken “voluntary” leave. I really hope this is voluntary. Errors can occur but someone should not get fired unless they are grossly negligent or fail to learn for previous educational measures. The system itself seems flawed as why would pharmacy be making batches of heparin flush…this is a big no no. Unless, they were forced to do it because of a heparin shortage. Either way, the pharmacy policies need to be revisited instead of punishing individuals.
