You’re fired, one more time

With minor-age patients, the parent is inextricable from the patient. Compliance and trust depend on a good relationship with both, and if that is missing with either the parent or the child, the therapeutic relationship is compromised. In those cases, I have found it best to suggest finding a new doctor somewhere else.
