The Healthcare Intelligence Network is tracking the application of the medical home model via the Medical Home Monitor portal at http://www.hin.com/medicalhome/medicalhome.html. There are white papers, audio interviews with early adopters (both payors and providers), surveys on industry- wide medical home adoption, a video on results from two medical home pilots for diabetes patients, plus a wide range of resources for those who want to learn about this model.
