She sounds like the second wife of every surgeon I know. She probably runs a spa and needs someone to take care of everything else. Ironically I find these creatures tend to inhabit suburbia much more than more established neighborhoods. We hire nannies, housekeepers, caterers and groundskeepers, as well as personal assistants. Au pairs are too young and have an other motives for being in the US; child care is a means to an end. A few years back a dual-doctor family lost their child after injuries sustained while in the care of an au pair.

Ironically, the other person that fits your description of the LTOH is many a Medicaid patient (minus the au pair of course).
