He seems to forget that he in fact works for the patients and he should publish his fees for the very simple reason that they his customers have the right to know. <br/><br/>It is a shell game that he is consenting to play. He can always decide what he ought to charge, collect it from his patients, and let any "games" go on between them and their insurer.
