We have a very nice building (leased from the hospital) and a fairly high overhead.&lt;br/&gt;&lt;br/&gt;It may seem like Pollyanna to you, but it is not made up.  We are really doing this.  If you have all capitated patients, then maybe it would not work, but if your HMO is a modified fee-for-service, then you can do what I do.  Just run your business well. &lt;br/&gt;&lt;br/&gt;Again, it may seem impossible, but it is no exaggeration.  Honest.</description>
