1. They all seem to be assuming preventive care saves money. I’m not aware of any significant evidence of this.

2. NOTHING is mentioned about the demand side of the equation. Until most of the patients are playing with their own money, the complicated rules, waits, and other forms of indirect rationing will get worse, no matter whether it’s one third party payer or many.
