They have public hospitals and private facilities. Medical licensure can be unrestricted, like most of have in the USA, or it can be a limited license. Limited, for example, to just one hospital. Usually the public hospital.

So if the public hospital exasperates the local Australian-trained docs, they leave for the private hospitals. The public hospitals find docs from other countries, but they are brought in on a limited license. Their license is limited to that public hospital.

Thing is, their visa may also be limited to that job. So, if you don’t like that public hospital, you can quit. But you can’t just go across the street to the private hospital. In fact, you no longer have a license, which means you no longer have a visa, and you have to leave the country.
