How hackable are they? &lt;br/&gt;&lt;br/&gt;And acknowledges that much of the information in the note is misleading because it is boilerplated day to day.  A concise handwritten (or even typed if not cut and paste) note is more useful.&lt;br/&gt;&lt;br/&gt;And I wonder how reliable the notes are when made from home.  Do you really remember what Mr. Jones reported and what his lungs sounded like?  Did you really look at his feet today, or just think that you did?
