What you’re leaving out is that if you ask such questions, you’re likely to be instantly labeled as “difficult” and in a few days you’ll get a certified letter: “Based on past interactions, I believe it is best for all parties involved that you seek care elsewhere.” I agree, the “10 Questions” are wonderful, but you’re rolling the dice if you ask them. And to anyone who responds “You wouldn’t want such a doctor anyway”…that depends entirely on whether your insurance will cover out-of-network costs to the extent you’ll need it to.
