I've been doing this a long time. I know the key words needed that "should" get my claims processed. I know exactly how to fill dictate a patient record so it conforms to medicare billing guidelines. I also have one person who does this as her ONLY job every day. She's really good (and she has a pretty low opinion of most people on the other end of the line ...from experience). Every newbie doctor figures it out within a year after finishing residency or they wouldn't survive the claims process. Part of the problem is that the insurance company's hire people who have no understanding of medcine whatsoever. One word…Bull$hit.
