In my field of psychiatry, the new practitioner will invariably get absolutely no-win cases, which he or she will naively accept.

As a new guy in town, I really got tied up with this nasty sticky situation. I asked an older colleague for advice. "What would you have done?" I asked. "I wouldn't have taken the patient."

I wonder if maybe the new surgeon is plagued by the same problem--a desire to prove himself that warps his judgement just enough that he jumps into something that his gut was telling him (or will be next time) that he should be more circumspect about.

Actually, is the observation and reporting that are different in the newscomer's situation. The new guy in town is always under the microscope. The established guys contniue to practice well or badly as they always have, it's simply that no one is on the lookout.
