All service calls to your home cost more than in the providers place of business. It takes time to drive there and back and there are time inefficiencies once he gets there as well.

I paid a TV repairman to fix my big screen. 80$ for the service call plus 70 for the labor while he was here. (plus more for parts). The fee for making the service call was more than the labor charge for the actual work.

I just paid a bill for the Air Conditioning guy to come out on a weekend. 210 $ hour door to door (his door).

Since you don't understand, you will not be using his service. If someone did that here in my town, I would jump on it--and have him come to my office.

I just paid a guy 150$ for a physical and checkup on a minor chronic problem. I had to be away from my business for 2 1/2 hours to make that appointment which cost me an additional $350. I would have been way ahead paying some internist $250 to come and see me in the office.

For some it isn't such a simple financial calculation, but just the comfort of being able to be called on in the privacy of your home instead of fighting traffic, parking, subways whatever to sit in the waiting room, etc.

Of course. It seems that he charges more for a visit than a traditional office based doctor does? A $200 appointment?

I don’t understand.
