how didn’t anyone notice the injury when she was 4? didn’t they see the puncture and wonder what happened?

i thought my missing car keys were bad

Maybe I misunderstand, but my take on the story was they full well knew what had happened, but thought it was not safe to remove the pencil. There was less risk to just leave the pencil where it was.

At the time, that may well have been true.
