All it is is a simple way to check off the required two “bullet point” elements of one of the required organ systems in order to meet the idiotic Medicare billing requirements.

Skin: warm and dry
General: alert, NAD (no apparent distress)
Etc.

Thank you, federal government, for making us check off a bunch of unnecessary extra boxes so we can get paid for our services.
