A doctor shortage is forcing this ER to close during certain hours:
Jane Bishop, president of the Sunshine Coast Medical Society, said there are only enough doctors to cover 66 of the hospital’s 88 monthly emergency-room shifts. Unless more physicians are found quickly, she said, St. Mary’s ER may have to be left unstaffed during traditionally slow mid-week daytime shifts.

I was going to say that the increased demand would be good for the new EM doc’s salary, but this ER is in Canada. I’d bet the salaries are pretty fixed by their socialized care.
No…it is fee or service, sometimes a salary is negotiated, depending on the situation.
