The most important medical advance is the double-blind randomized test. All other advances are secondary.

Ditto. Followed by the YAG capsulotomy. It’s laughable that evidence-based medicine is even on the list.
Sanitation, obviously. Without modern plumbing cities are essentially large sewers. No amount of vaccination or antibiotics can cope with that.
