The woman thought that mammograms prevent breast cancer rather than detects it early. (I wonder where she got this idea? Could it be because of some TV program?)
If she cannot afford to go to a doctor, how is she going to afford the cost of treatment? Or even simple evaluation of false positives which is a very real probability? In order for mammograms to reduce one’s chance of dying from breast cancer, one must have access to treatment and follow up.
I was always wondering about those “free mammograms” clinics. If a person truly doesn’t want mammograms, the money is not the issue. If the money is the only reason a particular woman doesn’t have mammograms, how would she pay for follow up if necessary?

She’ll go to a “city hospital” where she’ll get the care for free. Then when she dies anyway, because no treatment cures 100% of patients, her family will angrily sue the doctor who works in the free clinic, thus putting the healthcare system deeper in the hole.

The standard of care for following up on test results is a substantial burden and risk for any doc. I cannot blame the hospital/radiologist for this.
Likewise, I won’t draw any labs on kids in the OR while anesthetized that are for conditions beyond the scope of anesthesia practice. If the kid has Down syndrome and is an impossible stick, yet needs his thyroid function test, I’m sorry; there’s just no mechanism in place like in primary care offices to follow up with this lab result. No way am I going to get involved in the management of a chonic condition when I will never see the patient again (except in court!)
