flea,

From just what I have heard on the radio, I don't know the specifics or if he really had a heart attack. I have seen cases where buckshot has been able to enter the venous system and embolized at a later time. Theoretically, if there was a patent foramen ovale it can enter the left side and embolize to a coronary artery??

Anonymous, apparently they did all those things. They said the metallic BB, however, causes an image artifact which prevents it's precise localization.

I'm just a stupid pediatrician, so could one of you smart guys out there explain to me how a piece of buckshot that "migrates to the heart" causes a heart attack?

It's not DM to do a thorough survey on a 78 -year old who's just been shot.

Flea

I would probably have ordered a CT of head , neck, chest, abdomen and pelvis and this would have taken only a few minutes. Had this been done, they would have realized that there is a pellet in his heart that resulted in a "minor heart attack". Not that it would have changed the outcome. One more good reason to practice defensive medicine.
